(Maybe) Get sample data at http://qgis.org/downloads/data/

Add the cases table from postgres. Add the PLACE shp file from Tiger. Change color on the layer.

Filter to housing cases. Change color on dots to be a range by year (as category). Make heatmap with transparent start color.
