Include Tool Calls in Context Window?

I feel like I must be missing something incredibly obvious, but if I have tool calling enabled using useChat and streamText, the actual output of those tools that isn't captured by the message itself isn't passed back to the model. I'm calling a few different tools over a few steps and would really like future context to include these outputs. I have an incredibly hacky solution where I'm loading the tool results as message content from the db each time, but it feels like I can't be the only person dealing with this.
