Small tweak to Scorecards web page titles

From Isaac:

currently our page titles for the Scorecards are the same

So Plan Scorecards is Council Name | Council Climate Scorecards
And 2023 Action Scorecards is Council Name | Council Climate Scorecards

Could we change this to be Council Name | Council Climate Plan Scorecards with maybe a slightly updated meta description

And then when the new Scorecards come out we could name the previous ones Council Name | 2023 Council Climate Scorecards?
